By signing this form, I agree to utilize the rehearsal space within Image Dance Company & Academy's (IDCA) Studio space with courtesy for other rehearsing artists, parties and office personnel. Music will be played in an according manner to accommodate my necessary volume and tone for project I am rehearsing. Only water is allowed on the dance floor rehearsal space area. All food and/or other beverages will be placed in the reception area. No pets allowed in the studio. Any items moved or relocated within the studio space must be returned to same condition and placement. Any relocation of items must be approved by staff or administration member of IDCA. Rental MUST be PREPAID. If additional time is noted, then I will be charged before leaving premises. **SIGN BELOW *

Release of Liability: As the legal party, parent, guardian, renter, I release and hold harmless Image Dance Co. & Academy (IDCA), its's owners and operators from any and all liability, claims, demands, and cause of action whatsoever, arising out of or related to any loss, damage,or injury, including death, that may be sustained by the participant, onlookers, bystanders and/or the undersigned, while in or upon the premises or any premises under the control and supervision of IDCA, it's owners and operators or in route to and from any of said premises. * SIGN BELOW *
